Hardest of the Best: TSK's Tough Trivia Smackdown

This is my 101st quiz, and I decided to use my first 100 in the composition of a new work. I originally intended to randomly select one question from each of my 10 most popular quizzes based on average taker rating. As it happened, I had three quizzes tied for tenth place, each with an average takers' rating of 8.89, so I decided to expand this quiz to 12 questions. I than decided that rather than randomly select a question from each of these popular quizzes, I would select the hardest question from each such quiz determined by the number of people who got it wrong. All of the quizzes represented in this trivia smackdown are Editors Award winners so this is a challenge of some of the hardest questions appearing in some of my best and most popular quizzes.
Each question provides introductory information about its source and how difficult it has proven to be in the quiz in which it originally appears. The questions have been edited to some degree either to improve the writing of the original or simply due to the context in which the question originally appeared compared to the context in which it is now appearing. All editing changes are non-substantive, and none of the original answer choices have been changed.
The image used in this quiz is of the Eagle Scout medal, which introduced my quiz "Famous, Infamous and Notable American Eagle Scouts" - one of the quizzes represented in this trivia smackdown.
